Birth and Death Records

The Vital Statistics division of the Health Department issues birth and death certificates for those persons who were born or have died in Henry County since 1909. We also have books that contain birth and death record information available for public viewing for genealogy studies.

Immunization hours for childhood vaccines and adult vaccines are ever Wednesday:

Nursing Hours

Nursing hours are available Monday through Friday 1:30-4:00p.m. and Monday mornings from 9:00a.m.-Noon.

Services offered during this time include: TB skin testing, adult immunizations, free blood pressures, head lice checks, pregnancy tests, birth control refills, free HIV testing

No appointment is necessary.

Dental Clinic

The following clinic services are available for residents: preventative exams, x-rays, cleanings, fluoride treatments, sealants, extractions, fillings/restorations, phlebotomies, and acute pain relief. Clinic services for uninsured residents are billed on a sliding fee scale based on their family size and income. Lab services (dentures, crowns, etc..) are also available for patients of the dental clinic but are not on the sliding fee scale. However, through the Non-Profit Warehouse Dental Lab Program, patients of the dental clinic receive a considerable price break. Payment for clinic and lab services are required the day of service and before lab items are ordered. For patients on Medicaid, their services are covered in full and are not charged for services.

Women Infant and Children (WIC)

WIC is a nutrition education program. WIC provides nutritious foods which promote good health for pregnant women, women who just had a baby, breastfeeding moms, infants, and children up to age 5.

In order to qualify for WIC services, you must: live in Ohio, meet WIC income guidelines, and meet certain nutritional or health risks. WIC provides nutrition education, breastfeeding education and support, referral for health care, immunization screening and referral, and supplemental foods.

The Henry County WIC program is available on Mondays, Tuesdays, and Wednesdays. There is a morning clinic open on the 2nd and 4th Monday of each month and a late clinic is offered on the first Wednesday of each month.
